The Health Department:

Pauquachin First Nation's Health Team serves community members in reaching their highest state of holistic wellness through culturally supportive programs and services. We work with an 'any door is the right door' mindset to help community members access supports related to any of the Social Determinants of Health.

The Opportunity:

The Social Worker provides outreach services, support and crisis intervention to Pauquachin First Nation children, youth, and adults.

Duties and Responsibilities:

  • Provides referrals to external agencies, internal services, traditional holders and healers, and other culturally relevant resources;

  • Provides support to PFN members including assistance with legal challenges and obtaining legal support and legal aid, domestic violence and housing crisis supports and support to families dealing with the Ministry of Children and Family Development (MCFD) or other related government agencies, as well as provide on-one-one support to members navigating complex systems;

  • Assists in the development and facilitation of support and prevention groups for adults, children, and youth at PFN;

  • Delivers consultation, coaching, education and information to individuals, families, community, and caregivers about health and wellness, as well as external and internal resources;

  • Offers services such as assessment, therapy and crisis support, intervention and prevention for individuals on and off reserve and in the event of a community based critical incident, as well as consultation with outside agencies, schools, and hospitals;

  • Works to immerse cultural ceremony, tradition, and customs into and around related duties where appropriate and as needed:

  • Collaborates with Health Team, Traditional Healers and Elders, families, and community resources as needed to ensure prevention and healthy family development;

  • Develops, recommends, and implements practices and procedures to improve quality access to external resources available to community and provides advocacy to address barriers of service;

  • Promotes and maintains effective working relationships with members and families, physicians, schools, and hospitals to coordinate services;

  • Resolves conflicts, influences outcomes and encourages healthy interactions between community members and outside resources;

  • Ensures client/family/community confidentiality and privacy in accordance with professional regulatory guidelines and following standards of practice and legal requirements.

  • Case management, including providing therapeutic interventions with couples and families in collaboration with the Mental Health and Addictions Counsellor;

  • Assesses clients using a variety of methods, including psychometric tests, interviews and direct observation of behavior; creates supportive and meaningful therapeutic planning with families;

  • Work collaboratively with the Health Team to provide health care services to clients with physical and/or mental health, substance dependence, concurrent disorder and chronic disease; and in collaboration with agencies/services in the community, wherever appropriate, for the purpose of consolidating and coordinating client treatment;

  • Coordinates, attends and/or facilitates workshops, healing groups and speaking engagements with the community and in the schools;

  • Evaluates treatment outcomes to ensure effectiveness of services through continuous follow-up plans and actions;

  • Collaborates with the Health Team to identify and address gaps, barriers and challenges of service delivery and offer solutions;

  • Mediates, negotiates and resolves conflict resolution when responding or assisting in emergency situations;

  • Identifies and pursues funding for programs and initiatives through existing and new funding sources;

What we require from you:

  • Bachelor’s degree in Social Work (BSW) from an accredited school of Social Work;

  • Current full registration with the BC College of Social Worker as a Registered Clinical Social Worker (RCSW);

  • Minimum of 5 years of recent, related experience in direct service, community-based healthcare delivery systems;

  • 2 years’ recent experience delivering supervised clinical services to related to addictions, mental health, family conflict, relationships and social barriers; or

  • An equivalent combination of education, training and experience

  • Experience delivering therapeutic models such as Somatic Attachment Therapy, Solution-Focused Therapy, and Family Systems Therapy;

  • Experience working on advanced and complex professional social work cases;

  • Experience working from a trauma informed, culturally sensitive perspective;

  • Experience working with First Nations people and communities;

  • Experience working within Indigenous Child Welfare Services;

  • Valid class 5 driver’s licence, reliable vehicle and a clean driver’s abstract and appropriate insurance is required;

  • Valid first aid and CPR “C” certificate required;

  • Clean criminal record check with Vulnerable Sector required.
